HOW DO I GET AN APPOINTMENT?​Health Professionals

In order to register your interest in an appointment we recommend that you have ready:

  • The patient’s goals & information about their medical history
  • Reports/summary letters from a treating physician e.g. past therapists/doctor
  • Your NDIS plan details, if applicable
  • Your Medicare referral – we accept Enhanced Primary Care Plans/Chronic Disease Management/Team Care Arrangement Plans
